Tell us about your trip so we can review availability, distance, passenger eligibility, and facility requirements. Submitting an inquiry does not reserve transportation.
If your trip can be accommodated, we'll send your total approved fare before you reserve. Qualifying local procedure rides begin at $65; applicable add-ons or extended-distance charges are quoted in advance.
Complete the Procedure Transportation Reservation Form and submit the $15 reservation payment at the end of the form. The $15 is applied toward your approved fare.
We'll send confirmation of your reserved transportation and the exact deadline for your remaining balance. Once the balance is received, you'll receive your final transportation details.
