TRANSPORTATION TERMS AND CONDITIONS

These Transportation Terms and Conditions apply to passenger transportation services provided by True Standard Logistics, operated by We’re All the Way Up LLC.
By submitting a reservation form and reservation payment, the customer acknowledges and agrees to these terms.
Service Scope
True Standard Logistics provides prearranged, non-emergency transportation in a standard passenger vehicle.
True Standard Logistics is not an ambulance, emergency medical transportation provider, wheelchair transportation provider, stretcher transportation provider, home-health provider, or medical-care provider.
Transportation does not include medical monitoring, clinical supervision, lifting, transferring, medication administration, post-procedure care, or in-home assistance.
Passenger Eligibility
Unless specifically approved otherwise in writing, procedure transportation is intended for adult passengers who can enter and exit a standard passenger vehicle without being physically lifted and who do not require wheelchair transport, stretcher transport, specialized positioning, or medical monitoring during transportation.
True Standard Logistics may decline or discontinue transportation when a passenger's condition or transportation needs appear to exceed the reasonable scope of standard passenger transportation.
If an emergency occurs, emergency services should be contacted.
Facility Requirements
Customers are responsible for confirming the discharge and pickup requirements of their procedure facility before submitting an inquiry.
Some facilities may require a driver to check in, remain onsite, identify themselves, return at a particular time, or satisfy other facility-specific requirements.
Acceptance of a transportation request is based on the information supplied by the customer. If the facility later imposes materially different requirements that True Standard Logistics cannot accommodate, service may need to be modified, rescheduled, or canceled.
True Standard Logistics does not guarantee that any healthcare facility will accept an independent paid transportation provider as satisfying that facility's discharge policy.
Availability Requests and Reservations
Submitting a transportation inquiry does not create a reservation.
True Standard Logistics will first review availability, trip details, distance, and applicable facility requirements.
If the trip can be accommodated, the customer will receive an approved fare, reservation instructions, and payment deadline.
To reserve approved transportation, the customer must complete the detailed reservation form and submit the required $15 reservation payment.
The $15 reservation payment is applied toward the approved fare.
Any remaining balance must be received by the deadline specified in the customer's fare approval or confirmation. Failure to pay the remaining balance by that deadline may result in release of the reservation.
Pricing
Qualifying local procedure rides are currently priced at the published local rate. Extended-distance trips, additional pickups, facility standby, same-day requests, and other services may carry additional charges.
Any applicable charges will be communicated before the customer completes the reservation whenever reasonably possible.
Pickup and Facility Delays
Procedure completion and discharge times are controlled by the healthcare facility and cannot always be predicted.
True Standard Logistics will make reasonable efforts to coordinate transportation around the estimated discharge time but cannot guarantee that a facility will discharge a passenger at a particular time.
Extended onsite waiting is not automatically included in the standard fare. Any agreed standby service is subject to availability and applicable charges.
Destination and Assistance
True Standard Logistics provides transportation to the agreed destination.
The driver does not enter private residences.
When reasonably needed, the driver may accompany the passenger to the exterior entrance and remain until the passenger has entered the residence or reached the agreed exterior entrance.
The driver does not provide post-procedure supervision, in-home assistance, lifting, medical monitoring, or personal-care services.
Customer Information
Customers are responsible for providing accurate and complete information regarding the requested trip, passenger transportation needs, facility requirements, pickup information, destination, and contact information.
Materially inaccurate or incomplete information may affect our ability to provide service.
Conduct and Safety
True Standard Logistics reserves the right to decline or discontinue transportation when continuing the trip would create an unreasonable safety risk for the passenger, driver, vehicle, or others, or when the requested service falls outside the scope of standard passenger transportation.
Delays Outside Our Control
Traffic conditions, road closures, weather, collisions, facility delays, emergencies, government actions, and other events beyond our reasonable control may affect arrival or transportation times.
We will communicate significant known delays when reasonably possible.
Personal Property
Passengers are responsible for personal belongings brought into the vehicle. Customers should promptly contact True Standard Logistics regarding property believed to have been left behind.
Communications
By providing a telephone number or email address in connection with a transportation request, customers authorize True Standard Logistics to use that information for service-related communications concerning quotes, reservations, payments, pickup coordination, changes, and trip completion.
Limitation
Nothing in these terms is intended to waive or limit rights or liabilities that cannot lawfully be waived or limited.
True Standard Logistics is not responsible for losses resulting solely from inaccurate information supplied by the customer, healthcare-facility policies or delays, or circumstances outside our reasonable control, except where responsibility is imposed by applicable law.
Governing Law
These terms are governed by applicable laws of the State of Arizona.
